Mobile Crisis Co-Responder Documentation Workflow

1. Search for client in EMR or by your case list.  You should be assigned already.  If not notify your supervisor or Crisis supervisor for support. 

2. Schedule the following service: Adult, CYF, or DS: Mobile Crisis Second (see below for example)

3. Once you hit begin service, fill out the co-responder note section (see below for example).

 4. Document your service here- please include description of support provided, purpose and response to provided support, and plan for follow-up

5. Hit complete and then complete again and sign and submit.  Note: Be sure the time on the Mobile Crisis Second Service is the time you and the screener spent with client and/or family together (confirm with screener).  If you stayed longer to support the client after the screener left that time does not go into the mobile crisis second note.
