Advanced Directive & COLST/DNR Workflow

This workflow is to be followed when adding it initially to the client's medical record, when updating yearly during the Annual Update, AND/OR when the client reports making a change to their AD/COLST that is already on file. 

  1. Log into Credible

  1. Schedule the service (AD and COLST/DNR Update Form)

  1. Initiate the Service by clicking “Begin Service”

  1. Fill out the form

  1. Type (select Initial or Update – this will determine which path to take moving forward)

  1. Initial: Is this the first time you are asking the client if they have an Advance Directive and/or COLST/DNR? Select this option.

  1. Did you ask the client Do you have an Advanced Directive and/or COLST/DNR?"

  1. YESanswer the additional prompts

  1. Does the client have an AD and/or COLST/DNR? Indicate what they have (AD/COLST/DNR) and the date of the most recent version they have provided you with.

  2. If YES (The client DOES have an AD and/or COLST)Provide information about the AD/COLST/DNR in the prompts that appear (Date of most recent version & if documentation is obtained for attachment to the service - see below)


  3. Documentation of AD obtained, for attachment to this service? Select yes or no, if yes, attach to the service. If no, obtain and have Admin scan into the service to have on record.*Do not forget to obtain a physical copy of the AD/COLST/DNR to attach to the service.

  4. If NO (The client does NOT have an AD and/or COLST): Select one of the following:


  5. When selecting, be sure to have the dialog with the client about if they would like support in obtaining an AD/COLST (we can help get them started) or provide them with a handout to get started themselves.

  1. NO, client’s mental condition or behavior do not allow for a meaningful exchange of information. Provider will notify appropriate Admin staff to check the Vermont Advanced Directive Registry.

  1. If this option is selected, click NEXT to complete the form. This will then go to Admin (via multi-stage approval) to check the registry.  They will then record it in the form itself.

  1. Update: Is this an update that you are making to an existing Advanced Directive and/or COLST/DNR on file or updating at the time of the Annual Update? Select this option.

  1. Have you made any update to your AD/COLST?

  1. YES – answer the additional prompts

  1. Does the client have an AD and/or COLST/DNR? Indicate what they have (AD/COLST/DNR) and the date of the most recent version they have provided you with.

  1. Documentation of AD obtained, for attachment to this service? Select yes or no, if yes, attach to the service. If no, obtain and have Admin scan into the service to have on record.

  1. NO: If available, please reattach the most recent AD/COLST/DNR in the system to this service.

  1. Click Next to save

  1. Click Complete to Sign and Submit

  1. Sign & Submit the service.
